Hospice Social Worker

Full Time - Serving South Denver Area

This high-energy, flexible and congenial person is responsible for providing practical, psychosocial and emotional support to hospice patients and their families. The hospice social worker serves as advocate for the patient and family, collaborating with the interdisciplinary team and providing ongoing assessment and updates as a patient's condition changes. 

 Job Responsibilities (include, but are not limited to):

  • Assisting the members of the IDT in understanding the significance of psychosocial, emotional, and financial factors related to the patient and family during and after the terminal phase of illness, as well as the effects of caregiving.
  • Educating clients on decision making and advance directives. Provide materials and assist with completion as necessary.
  • Has thorough knowledge and understanding of the Medicare Hospice Benefit as well as Medicaid and private insurance benefits and options for the uninsured. Educates patients, families, the IDT, and community members as requested.
  • Utilizing Medicare guidelines when admitting, recertifying, and discharging patients. Follow relevant criteria when determining level of care.
  • Participating in patient admissions as requested.
  • Facilitating emergency, temporary, or permanent placement. Complete PASRR as needed.
  • Maintaining proper and timely records of visits, contacts, assessments, and interventions with patients and families. When services are provided in a facility, copies of all documentation will be left in the facility charts per facility protocol prior to leaving the facility.

The social worker will adhere to the privacy practices as described in the Notice of Privacy Practices and as required by the privacy rule of the administrative simplification provisions of the Health Insurance Portability and Accountability Act of 1996. Perform all duties in compliance with Agape Hospice Services’ policies and procedures.

-Masters of Social Work (MSW)

-Two years experience working in hospice care or a related field

-Valid Colorado driver's license, proof of auto insurance and reliable transportation.
