By Your Side: Another Level of Care from Agape
When symptoms become unpredictable or difficult to manage, Agape’s By Your Side program is here to help. By Your Side provides intensive care at home as part of hospice treatment. Our expert team will be there when you need us, and will stay until symptoms are under control.
By Your Side Care is:
- Temporary additional care that treats difficult-to-control symptoms at home.
- A higher level of care that continues until symptoms are under control.
- Care that is provided in addition to other regular home healthcare visits.
- Part of hospice treatment.
- 100% covered by Medicare and most Medicaid and private insurance.
- A benefit with few, if any, out-of-pocket costs.
- A hospice benefit that offers support and training to the patient’s caregivers.
- Another way Agape Healthcare helps hospice patients live out their last days in comfort and with dignity.
If you are interested in registering for the By Your Side program, please call us at 877-771-1231. Once registered, the By Your Side team is available immediately to the hospice patient and his/her family. The patient’s primary care nurse will consult with the By Your Side team to determine the best treatment options.
